MEDICAL RESPITE SOCIAL WORKER
Community Health Centers of Burlington
Community Health Centers of Burlington is seeking a Medical Respite Social Worker (MRSW) to be a liaison between the hospital and the medical community for all referrals to, and placements in, the medical respite program. The MRSW will work with hospital staff, as well as other referrals sources to ensure safe, appropriate medical respite placements for optimal patient recovery and with the goal of avoiding the need for (re)hospitalization. This position will be in communication with a large variety of providers including, but not limited to hospitals, out-patient care and home health agencies as well as human service agencies serving the homeless. Skills & Requirements
- MSW Degree from graduate school of social work, accredited by (CSWE).
- Minimum of two years of medical social work experience required.
