Responsibilities:

• Provide skilled PTA services under the supervision of a licensed PT 
• Carry out individualized treatment plans in patients’ homes 
• Document patient progress accurately and timely 
• Communicate effectively with supervising PTs, physicians, and caregivers 
• Help patients improve mobility, safety, strength, and independence 

Requirements:

• Active New Jersey PTA license 
• Reliable transportation and valid driver’s license 
• Home care experience preferred, but not required 
• Strong communication and time management skills 
• Passion for patient-centered care 

Service Area:
Essex County, NJ — including Livingston, West Orange, Verona, Cedar Grove, Bloomfield, Glen Ridge, and Montclair.
