We're committed to creating a workplace where collaboration, compassion, and professional excellence come together to make a difference every day.How You'll Make an ImpactImplement individualized treatment programs developed by the supervising Physical Therapist.Support residents in reaching their rehabilitation goals through therapeutic exercises and functional mobility training.Monitor resident performance and communicate progress, concerns, and recommendations to the therapy team.Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.Collaborate with nursing staff and interdisciplinary team members to promote successful outcomes.Encourage resident participation while fostering confidence, safety, and independence.What We're Looking ForGraduate of an accredited Physical Therapist Assistant program.Active Nebraska Physical Therapist Assistant license.Strong clinical, communication, and organizational skills.Ability to build positive relationships with residents, families, and team members.Desire to provide person-centered care with professionalism and compassion.Why Heritage Crossings?When you join Heritage Crossings, you're joining a community that values both its residents and its employees.
