Behavioral Health Care Coordinator

What You'll Do Care Coordination & Client Support Coordinate comprehensive case management services for Behavioral Health clients. Assist clients with housing stabilization, discharge planning, and access to community resources. Connect clients to recovery-oriented services, housing programs, financial resources, and supportive services. Develop individualized service plans, monitor progress, and adjust care plans as needed. Encourage and support client participation in recovery and wellness programs. Collaboration & Advocacy Serve as a liaison between clients, treatment providers, community agencies, and other stakeholders. Coordinate referrals and ensure continuity of care throughout the recovery journey. Advocate for clients to help remove barriers to treatment, housing, and essential services. Participate in multidisciplinary treatment team meetings and aftercare planning. Documentation & Compliance Maintain accurate and timely client documentation in HMIS/Credible and other required systems. Prepare and review client records to ensure compliance with licensing standards and organizational policies. Monitor documentation for accuracy, completeness, and quality. Community Engagement Build strong relationships with community providers, housing organizations, and recovery partners. Stay informed about community resources and emerging behavioral health trends. Educate clients and community members on substance use disorders (SUD), co-occurring disorders, and available recovery resources. Safety & Professional Development Help maintain a safe, clean, and welcoming environment for clients, staff, and visitors. Participate in emergency drills, staff meetings, and ongoing professional development. Complete additional duties that support the mission and success of the Behavioral Health program. What We're Looking For Required Qualifications High School Diploma or GED. Minimum of 2 years of case management experience . Strong organizational, communication, critical thinking, and problem-solving skills. Working knowledge of behavioral health, addiction, and recovery principles. Proficiency with Microsoft Office (Word and Excel). Valid driver's license. Preferred Qualifications Bachelor's or Master's degree in Social Work or a related human services field.
