Position Overview: We are seeking a compassionate and licensed Registered Nurse to coordinate and deliver home care and hospice services in a supportive and community-focused environment. This role involves planning and directing care based on physician treatment plans, ensuring patients and families receive personalized attention within their homes and surrounding communities in the Midwest region.

Key Responsibilities: The nurse will organize and manage home health and hospice services, collaborate with community resources, and may participate in public health initiatives. Additionally, the nurse will serve as a case manager for hospice clients, providing holistic care and support.

Education: Graduation from an accredited nursing program is required. Current licensure as a Registered Nurse is mandatory.

Experience: While two years of professional nursing experience is preferred, it is not a strict requirement. The ideal candidate is self-motivated and capable of working independently.

Requirements:

• Valid Registered Nurse license in the applicable state.
• Current CPR certification.
• Professional demeanor suitable for interacting with clients, visitors, and healthcare personnel.
• Possession of a valid driver's license and proof of motor vehicle insurance.
