SUMMARY

Family Service League is seeking a full-time Social Worker for the Family Recovery Center in Bay Shore. The Social Worker would provide assessments, direct care, and facilitation of referrals to individuals who are struggling with mental illness, substance use disorder, or a dual diagnosis. Additionally, the Social Worker would provide support to the family members who are being impacted by their dependency. The Family Recovery Center offers both individual and group treatment models best suited for the clients’ needs. RESPONSIBILITIES

The Social Worker will manage a caseload of clients and provide case management services. Conduct intake assessments. The Social Worker will provide individual and group therapy to adults, adolescents, and children with the full range of DSM diagnoses, including both mental health and substance use disorders, in an acute care, outpatient setting. Periodic, field-based work required. Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record (EMR). The Social Worker will follow up with clients during the stabilization period and immediately following. Maintain up-to‑date progress notes, treatment plans, and Outpatient Treatment Reviews (OTRs). Coordinate with outside providers and collateral contacts. Attend routine supervisions and team meetings. Coordinate treatment with a multidisciplinary team. Complete training and maintain proficiency with de‑escalation and physical restraint skills. All other duties as assigned.

QUALIFICATIONS

Master’s degree in social work required. LMSW, LMHC, LMFT, CASAC, Limited Permit Social Worker or related licensed clinical degree required. At least 6 months of experience in a behavioral health setting required; clinic experience preferred. Knowledge base of substance use disorders preferred. Ability to problem solve, make clear decisions and work as part of a team. Excellent interpersonal, verbal, and written communication skills required. Proficient computer skills, including Microsoft Office and Electronic Health Records required. Some evening and weekend hours may be required.
