Title
Social Work / Case Management - Neuro and Cardiac Care

Schedule
- Weekend coverage: Minimum 2 weekend days per month.

Requirements
- Minimum 1 year inpatient/acute care and case management experience.
- Prior hospital experience required.
- Epic charting experience required.
- Required certifications: BLS, LSWAIC.
- Additional credentials: LCSW / MSW preferred.

Roles & Responsibilities
- Coordinate care for neuro and cardiac patients.
- Conduct biopsychosocial assessments and provide crisis intervention.
- Manage discharge planning and referrals to rehabilitation or long-term care.
- Facilitate prior authorization and documentation; coordinate with home care and durable medical equipment providers.

Compliance / Notes
- References: 2 references covering 1 year (within the last 3 years) required.
- Pre-start modules (~16 hours average) and annual updates required. Completion recorded in transcripts.
- COVID-19 vaccine NOT required; flu vaccine status unknown.
