Registered Nurse/Licensed Practical Nurse — AIMED

Responsible to: Chief Program Officer

General Description:
Provide nursing services to individuals with intellectual and developmental disabilities (IDD) in residential and community-based settings. Deliver direct clinical care, medication management, health assessments, care coordination, staff training, documentation, and advocacy while ensuring compliance with physician orders, agency policies, and applicable regulations. Renders other nursing care as prescribed by a physician or as identified by client needs, within scope of practice; and implements, with other direct care staff, appropriate protective and preventive health measures. Leads in the training/orientation of new nursing staff.

Residential group home nurses manage healthcare for individuals in community-based houses, operating with high autonomy and navigating a blend of domestic and medical environments.

Core Clinical Responsibilities:

• Provide routine and emergency nursing care, including medication administration, blood glucose monitoring, insulin administration, vital signs, wound care, and other treatments within scope of practice.
• Monitor and manage chronic physical and behavioral health conditions; identify changes in health status and notify physicians and program leadership as appropriate.
• Conduct nursing assessments, annual health screenings, Health Risk Screening Tool (HRST) clinical assessments, and ongoing health monitoring.
• Develop, implement, review, and update nursing components of Individual Support Plans (ISPs) and medical care plans.
• Develop and implement protocols, procedures, and policies concerning medical to ensure the health and safety of individuals supported by AIMED
• Coordinate and schedule routine, quarterly, annual, and follow-up medical appointments in accordance with regulatory requirements.
• Maintain accurate medical records, medication records, physician orders, nursing tracking forms, and health documentation in Therap.
• Attend interdisciplinary and multi-disciplinary team meetings to ensure team understanding of comprehensive medical needs of all individuals supported. 
• Review hospital discharge summaries, laboratory results, physician orders, and other medical documentation; communicate follow-up needs to staff.
• Develop and implement medical protocols in collaboration with physicians and other healthcare professionals.
• Monitor medication errors, maintain medication accountability, and ensure accurate administration and documentation.
• Conduct Medication Administration Training and Practicum Observations to ensure staff certifications concerning medication administration
• Coordinate care with physicians, pharmacies, therapists, families, advocates, and other healthcare providers.
• Renews physician’s orders and review for accuracy, including monitoring, discontinuing, and ordering medications in conjunction with the physician orders.
• Ensuring prescriptions are current and on site. Transporting medications to sites, as needed. 
• Participate in clinical treatment teams and admission/discharge decisions based on medical needs.
• Assist with infection control, communicable disease prevention, first aid, and emergency response training of staff.
• Provide 1:1 nursing supervision and on-call medical support as needed.
• Ensure medical best practices, cultural sensitivity, and individual rights are always adhered to.
• Provide the CEO or designated person with a monthly report on an individual's care plan (needs, updates, and/or changes).
• Other duties as assigned 

 

Day-to-day practice:

• Home-Based Care: Operating outside clinics, such as the office or the individual’s home.
• Medication Oversight: Audit home medical logs and ensure strict state compliance.
• On-Call Duties: Answer urgent phone calls for medical triage decisions.

 

Staff, Family & Community Collaboration:

• Train and educate Direct Support Professionals (DSPs) on medications, medical protocols, infection control, health monitoring, and emergency procedures.
• Provide remediation and health/hygiene training for participants and staff.
• Collaborate with families, advocates, therapists, and healthcare providers to promote continuity of care.
• Family Liaison-Guide parents through complex medical care and transitions or aging milestones in accordance with the agency's policies and procedures.
• Advocate for client health, safety, rights, independence, and community inclusion.
• Participate in ISP meetings as a healthcare professional and provide medical recommendations.

Qualifications & Requirements:

• Must have a current Pennsylvania RN or LPN license and CPR/First Aid certification.
• Experience: Graduate of an approved nursing program. A minimum of 3 years of healthcare experience (acute, long-term care, MH/MR), including at least 2 years as an RN/LPN; IDD experience preferred.
• Must have a valid driver's license and acceptable driving record.
• Minimum age of 21
• HRST Clinical Reviewer, or ability to achieve certification
• Medication Administration Train-the Trainer, if applicable
• Must have excellent knowledge of nursing practices, treatment planning, and healthcare needs of individuals with IDD/MH.
• Must possess strong clinical judgment, documentation, communication, organizational, and interpersonal skills.
• Ability to work independently with limited supervision and direct/support lower-level staff.
• Exceptional writing skills and the ability to prepare and maintain accurate medical records. 
• Physical/Active support: Ability to lift/carry up to 70 lbs. and provide physical assistance when required.
• Clearances: Employment is strictly contingent upon successfully clearing PA ACT 33 (Child Abuse), Act 34 (Criminal), and Act 73 (FBI fingerprints) screenings.
