Licensed Practical Nurse (LPN) – 2nd Shift

Location: Toledo, Ohio
Position Type: Full-Time
Schedule: 2:00 PM – 11:00 PM
Weekend Requirement: Every Other Weekend

Position Overview

We are seeking a compassionate, dependable, and experienced Licensed Practical Nurse (LPN) to provide quality nursing care for individuals with intellectual and developmental disabilities.

The ideal candidate will have strong clinical skills, experience administering medications and treatments, and the ability to work collaboratively with registered nurses, physicians, and other healthcare professionals.

This position involves providing direct patient care, monitoring patient conditions, administering prescribed medications, maintaining accurate documentation, and ensuring patient safety and comfort.

Key Responsibilities

Nursing Care & Medication Administration

• Provide direct nursing care within the LPN scope of practice under the direction of an RN or other authorized healthcare provider.
• Administer prescribed medications, including oral, topical, rectal, and authorized injectable medications.
• Administer oxygen and assist with respiratory treatments.
• Perform wound care, sterile dressing changes, and other prescribed nursing treatments.
• Insert and maintain catheters, perform suctioning, and provide other authorized nursing procedures.
• Assist physicians and healthcare providers with examinations, treatments, and diagnostic procedures.
• Monitor patients for medication reactions, treatment effectiveness, and changes in medical conditions.
• Respond to medical emergencies and provide first aid or CPR when necessary.

Patient Monitoring & Direct Care

• Monitor and record vital signs, including blood pressure, temperature, pulse, and respiratory rate.
• Observe and report changes in patients' physical, emotional, and behavioral conditions.
• Assist patients with activities of daily living, including bathing, dressing, grooming, feeding, and mobility.
• Reposition patients and assist with maintaining their comfort and safety.
• Respond promptly to patient requests, alarms, and medical needs.
• Assist with patient appointments, activities, and treatment plans.
• Provide compassionate care while maintaining patient dignity and confidentiality.

Documentation & Medication Management

• Maintain accurate patient medical records, nursing notes, and medication administration records.
• Document patient assessments, treatments, medications, and responses to care.
• Maintain medication accountability, including medication counts and inventory.
• Monitor medical supplies and equipment to ensure availability.
• Complete incident reports and other required clinical documentation.
• Follow HIPAA regulations and established healthcare policies and procedures.

Teamwork & Staff Support

• Collaborate with registered nurses, physicians, and interdisciplinary healthcare teams.
• Provide guidance and assistance to nursing assistants, patient care aides, and other direct care staff.
• Assist with orientation and training of support staff as assigned.
• Participate in nursing meetings, staff conferences, and continuing education programs.
• Communicate effectively with patients, families, and healthcare team members.
