The Case Manager provides comprehensive, client-centered advocacy and case management services to adult and child survivors of family violence, sexual assault, and human trafficking. This position works closely with survivors to assess needs, develop individualized service plans, provide safety planning, and connect clients to community resources that promote safety, healing, and long-term stability. 

Applicant must be willing to serve in rotation for 24-hour on-call response, including evenings, weekends, and holidays

Applicant must be willing to use a personal vehicle for work/ travel, have a valid driver’s license, a reliable vehicle, and current car insurance

Schedule/Availability: 40-50 hrs/wk. Evening, weekend, and holiday on-call required. Scheduling flexibility needed.

Job Responsibilities:

  • Conduct intake assessments and ongoing needs assessments.
  • Provide trauma-informed case management and advocacy services.
  • Develop individualized service plans with measurable goals.
  • Conduct danger assessments and personalized safety planning.
  • Assist clients with applications for Crime Victims’ Compensation (CVC).
  • Provide referrals to emergency shelter, housing, counseling, legal services, and community resources.
  • Assist clients with obtaining protective orders and navigating the legal system.
  • Accompany clients to court, law enforcement interviews, forensic exams, and other appointments as needed.
  • Coordinate emergency assistance, including hotel stays, transportation, and basic needs support.
  • Maintain accurate and timely documentation in the agency’s case management system.
  • Collaborate with law enforcement, hospitals, CPS, prosecutors, and community partners.
  • Participate in multidisciplinary team meetings and staff meetings.
  • Maintain confidentiality and comply with agency policies, funder requirements, and ethical standards.
  • Provide services in both virtual and in-person settings as appropriate.
  • Apply specialized training and use professional judgment in determining the best way to serve survivors during a crisis and on an ongoing basis.
  • Ensure case management in collaboration with ongoing system and service- or investigation-based case management, continuing case management beyond others’ eligibility parameters. Ensure that case management is carried out in a manner that is relationship-based, trauma-responsive, and survivor-centered.
  • Document all interactions with survivors, their families, and partner agencies within 72 hours.
  • Build and maintain good relationships with organizations that provide services relevant to survivors.
  • Alert supervisor when difficult situations arise.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or a related field preferred.
  • Candidates with relevant case management, victim advocacy, or other comparable human services experience are strongly encouraged to apply in lieu of a bachelor’s degree.
  • Minimum of one year of experience in case management, victim advocacy, crisis intervention, or related human services preferred.
  • Knowledge of domestic violence, sexual assault, human trafficking, trauma-informed care, and community resources.
  • Strong organizational, communication, and crisis intervention skills.
  • Ability to work with diverse populations in a compassionate, culturally responsive, and nonjudgmental manner.
  • Reliable transportation, valid driver’s license, and proof of insurance.
  • Ability to pass a criminal background check.
