Clinical Coordinator, Registered Nurse (RN) - Command Center

Overview VCU Health Continuum Integration Center (CIC) vision is to be a world-class, virtually enabled 24/7 care coordination command center that manages a holistic patient experience and ensures the best outcome for each individual, with the most efficient use of resources for the healthcare system. The CIC Clinical Coordinator collaborates with the nursing team to direct, monitor, and evaluate daily nursing care in accordance with established policies and procedures. This leadership role serves as a resource for VCU Health CIC command center and Hospital at Home staff and supports the organization\'s vision/mission/values to enhance patient experience and clinical outcomes. Programs within the CIC supported by this role include Hospital at Home, Remote Patient Monitoring (RPM), Ask-A-Nurse, and Post-Acute Nurse Triage. Note: The role requires travel between the Northside location and the VCU Medical Center campus, with possible travel to a patient\'s home (within a 25-mile radius). Key Functions Collaborates with subject matter experts and the CIC triad Leadership team (Medical Director, Nurse Manager, Sr. Director) to create an environment of teamwork that supports improved outcomes, service, and staff engagement. Leads daily IDT and sets staffing daily assignments in accordance with patient care needs and productivity guidelines. Supports clinical problem solving and medical escalations; covers for staffing needs. Assists with Hospital at Home and RPM program admission and discharge processes to ensure efficient throughput and high-quality, patient-centered care. Ensures quality in partnership with Hospital at Home\'s network of integrated services including but not limited to dietary, pharmacy, infusion, laboratory, imaging, therapy, transportation, durable medical equipment, and personal care services. Supports proper inventory control and assists with managing supplies, technology, and equipment; engages in problem solving (phones, technology, RPM equipment, etc.). Supports a patient-first philosophy and engages in service recovery when necessary. Supports CIC engagement by operationalizing current nursing strategies using employee rounding across multiple campuses and other initiatives. Participates in ongoing assessment of the quality of patient care services provided in the CIC command center, in collaboration with the management team. The Clinical Coordinator facilitates patient flow, coordinates activities and communication for the clinical area nursing unit during assigned shift. This person serves as a resource and leader for patients, staff, health care providers, administration and other disciplines. The Clinical Coordinator ensures that patient care is delivered in an efficient, caring and courteous manner. Licensing, Certification, or Registration Requirements Current RN licensure in Virginia or eligible compact state. Licensing, Certification, or Registration Requirements for Continued Employment Current RN licensure in Virginia; BLS/HCP certification or equivalent. Experience Minimum of three (3) years of clinical experience. Charge Nurse experience or supervisory experience. Preferred Experience Academic health care experience. Experience with similar patient population in either an acute care setting or as applicable to area of practice. Education / Training Required: Diploma or Associate Degree from a professional nursing program recognized and/or approved by the corresponding state\'s Board of Nursing. All RNs without a Bachelor\'s degree in Nursing (or higher) will be required to enroll in an approved RN to BSN program within two years of start date and complete within five years. Preferred: Certification in a specialty area; Bachelor\'s Degree (or higher) in Nursing from an accredited program. Independent Action Follows documented physician/licensed independent provider orders. Practices within the regulations governing the practice of nursing in the Commonwealth of Virginia. Practice guided by the ANA Code of Ethics for Nursing and established national nursing practice standards. Follows VCUHS policies and procedures and established practice guidelines. Organizes and plans work with input from the patient/family with specific outcomes and sound clinical judgment and resources. Supervisory Responsibilities Organizes work and delegates and supervises others (RNs, LPNs, Care Partners, Unit Secretaries, etc.) in carrying out assignments. Additional Position Requirements Clinical Coordinators work designated shifts, days of the week, and/or holidays based on work area. Individual shift rotation and schedules will be established with the supervisor. Schedules may be adjusted to meet unit requirements.
