Description Social Worker Care Management | Full-time, 80 hours bi-weekly Monday-Friday, 12p-8pm Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Ideal candidates thrive in interdisciplinary environments and are committed to improving patient outcomes through holistic, evidence-based care. Key Responsibilities: Coordinate psycho-social support for patients during hospitalization Develop and implement personalized discharge plans Assess patient needs related to care coordination, resources, and services Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction Facilitate access to community resources to support recovery and continuity of care Ensure compliance with hospital policies, regulatory standards, and evidence-based practices Qualifications: Education: Bachelor’s degree in Social Work required Experience: Minimum two years in acute care, SNF, physician offices, or community health settings Licensure / Certification: LBSW license in Maryland required Must obtain care/case management certification within five years of hire Skills & Abilities: Strong communication and multitasking skills Familiarity with EMRs and ability to navigate electronic medical records Knowledge of disease processes, regulatory requirements, and evidence-based care Understanding of community resources, provider networks, and the revenue cycle Ability to build relationships across clinical and community settings Caring for Our Team We are committed to supporting the well‑being and professional development of our team members through a comprehensive benefits package, which includes: Health, Dental, and Vision Insurance Coverage begins on the first of the month following your hire date.
