Key Responsibilities

• Perform comprehensive physical therapy and OASIS assessments, including psychosocial, educational, safety, and risk factor evaluations, in accordance with federal and state regulations and APTA guidelines
• Develop and implement individualized plans of care with patient-specific goals, skilled PT interventions, and patient/caregiver education to promote optimal functional independence and safety
• Coordinate patient care as Care Manager and/or team member, collaborating with physicians, nurses, caregivers, and other rehabilitation services to ensure optimal outcomes
• Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
• Complete and maintain accurate clinical documentation, OASIS time points, charging and billing paperwork, and required agency, state, and federal forms in a timely manner
• Actively manage and adjust patient visit schedules to maximize productivity and patient need, communicating changes to the scheduler and supervisor in a timely manner

Required Qualifications

• Active Physical Therapist license in the state of Vermont (or eligibility)
• Knowledge of and ability to perform OASIS assessments in compliance with agency, state, and federal guidelines
• Familiarity with federal and state home health regulations and APTA standards of care
• Ability to complete accurate clinical documentation and billing/charging processes
• Strong organizational and communication skills to coordinate care across multidisciplinary teams

Preferred Experience / Skills

• Prior home health experience with OASIS documentation
• Experience supervising PTAs, PT students, or home care aides
• Familiarity with McKesson or similar home health electronic medical record systems
• Experience with wound care, staple/suture removal, and PT/INR or blood pressure assessments in a home health setting
