Role Overview Responsible for evaluating and treating patients in a home care setting to restore function, reduce pain, and improve mobility using various therapeutic procedures. The Physical Therapist collaborates with healthcare team members to provide holistic, patient-centered care aligned with their State Practice Act. Role Overview Responsible for evaluating and treating patients in a home care setting to restore function, reduce pain, and improve mobility using various therapeutic procedures. The Physical Therapist collaborates with healthcare team members to provide holistic, patient-centered care aligned with their State Practice Act. Key Responsibilities Assess patient needs and develop comprehensive care plans. Implement therapeutic interventions to promote recovery and well-being. Collaborate with team members and communicate effectively with patients and caregivers. Utilize office equipment such as tablets and cell phones for documentation and communication. Travel within service area to provide in-home care. Qualifications & Skills Graduate of an APTA-accredited Physical Therapy program. Current license in the applicable state. Excellent communication and interpersonal skills. CPR certification required. Ability to assess needs, develop treatment plans, and demonstrate professional procedures in home care. Flexible availability, including weekends and holidays. Valid driver’s license and ability to travel within the service area. Additional Requirements Proficiency in applying physical therapy principles, methods, and equipment. Ability to work as part of an integrated healthcare team. #J-18808-Ljbffr
