Position Summary

The Speech-Language Pathologist will assess patient needs, develop individualized treatment plans, provide direct therapeutic interventions, document clinical progress, and collaborate closely with physicians, therapists, caregivers, and family members to support successful patient outcomes.

Key Responsibilities

• Conduct comprehensive evaluations for speech, language, communication, cognitive-communication, voice, fluency, and swallowing disorders, as appropriate.
• Develop individualized treatment plans based on clinical findings, physician referrals, patient goals, and established plans of care.
• Provide direct speech-language therapy services in accordance with professional standards and applicable regulatory guidelines.
• Clearly communicate treatment goals, expectations, and progress to patients, families, caregivers, and interdisciplinary team members.
• Maintain accurate, timely, and compliant clinical documentation within the designated documentation system.
• Monitor patient progress and modify treatment plans as clinically indicated.
• Educate patients and caregivers on home exercise programs, communication strategies, therapy techniques, and continuity-of-care recommendations.
• Collaborate with physicians, physical therapists, occupational therapists, case managers, and administrative staff to ensure coordinated patient care.
• Promote a culturally competent and inclusive care environment.
• Maintain compliance with all licensure, credentialing, privacy, and documentation requirements.

Qualifications

• Degree in Speech-Language Pathology, Communication Sciences and Disorders, or an accredited related program with appropriate certification.
• Active and unrestricted Speech-Language Pathologist license.
• Strong clinical assessment, treatment planning, documentation, and patient-care skills.
• Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare teams.
• Excellent communication skills, professionalism, and patient-centered approach.
• Strong organizational skills with the ability to manage caseload priorities and maintain timely documentation.

Preferred Qualifications

• Experience working with diverse patient populations.
• Familiarity with healthcare regulatory guidelines and compliance standards.
• Previous rehabilitation, outpatient, home health, or skilled nursing experience is a plus.
• Spanish-speaking candidates are preferred; however, bilingual proficiency is not required. Qualified candidates who do not speak Spanish will also be considered.
