Operating from a client centered perspective, Home/Life provides a safe and therapeutic environment that supports the social and economic independence of its families served. By developing and implementing innovative services and programs within our Families with Children’s facilities, we provide case management and job counseling, as well as help our families access the economic housing support that allows them to enter and stay in permanenthousing. Under the direction of the Case Manager Supervisor and Social Services Director, The Case Manager is responsible for overseeing client's cases from intake to move out, providing the support necessary to achieve the bestoutcomes. DUTIES AND RESPONSIBILITIES The responsibilities of the Case Manager include but not limited to: Administrative: Completes an assessment and fathers required documents (i.e.: birth certificates, social security cards, picture IDs,etc.) Shows the Safe Sleep Video and carry out the appropriate documentationprocess Makes sure that clients sign the Code of Conduct and the facilities rules andregulations Obtains any possible proof of employment and assists with obtainingentitlements Maintains clients’ records by reviewing case notes; logging events andprogress Communicates clients’ progress by conducting weekly interdisciplinary meetings andevaluations Handles intakes of adults and ensures that all documentation is accurate and properlyfiled Recommends action plan and develops an Independent Living Programs (ILP) for clients that is reviewed and updated on a weeklybasis Collaborates with the Housing Specialist to create an exit strategy and target exit date Conducts weekly unit inspections Maintains detailed records of each client in the form of progress notes throughCARES Provides Social Services Director (SSD)/ Case Manager Supervisor with reports and summaries of caseload for managementreview Complete new Intake assessment within 48 hours of family admission to thefacility Advocate and make referrals to agencies and service providers based upon familyneeds Liaison for management, communicating clients’ progress and any caseconcerns In the event of an emergency, Essential Employees are required to report to work as scheduled Programmatic Achievement: Carries out case management activities as per the Part 900 Regulations and the Department of Homeless Services (DHS) Case Management Guidelines for familyshelters Tasks family with seeking and acceptinghousing Responsibleformeetingtargetgoalssetforeach client bythe Housing Specialist Develop individualized short term and long-term goals andobjectives SKILLS AND QUALIFICATIONS Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, Human Services, or Education Servicesrequired.
