The LPN typically works under the direction of a Registered Nurse and provides nursing and facilitation services for assigned clients within each designated program. The LPN works as part of the clinical team to implement evidence‑based treatment strategies that promote physical, psychosocial, and spiritual health. The LPN also provides training to clients or family members on how to promote health, wellness, and medical stability. Essential Duties & Functions Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned client as evidenced by documentation and observation of positive client care outcomes. Identify individuals in need of medical nursing services or interventions in a timely manner and provide interventions as needed. Possess specialized knowledge and clinical skills necessary to provide care for clients; perform hands‑on nursing care and facilitator care using the nursing process. Train and educate families, funders, employers, and other constituents, and use appropriate resources to develop and implement an individualized teaching and discharge plan. May support or provide direct care to clients at program locations or during medical appointments as needed. Education & Experience Requirements State licensure as a Licensed Practical Nurse (or equivalent) is required. Experience in the rehabilitation of chronically or catastrophically ill or injured is recommended. Demonstrated accountability and skills in analysis, problem solving, decision making related to medical nursing skills and time management are required. The ability to communicate in writing and verbally with families, clients, staff and physicians is necessary. Possession of a valid driver's license and acceptable driving record must be maintained at all times.
