Job SummaryThe Adult Case Manager coordinates and oversees services for participants in the KIC Elders Program in collaboration with the Elder Care Givers staff. This position is responsible for case management, care coordination, and ensuring quality service delivery in alignment with tribal values and standards.This role supports elders by conducting assessments, developing and monitoring service plans, and advocating for services that promote independence, dignity, and cultural identity. As a part of an interdisciplinary team, the Adult Case Manager ensures services are delivered in a respectful, culturally appropriate manner that supports overall health and well-being. The position includes ongoing evaluation of care plans and providing guidance and oversight of care plans and providing guidance and oversight to caregivers to ensure services meet the needs of elders and tribal expectations.Job Duties & ResponsibilitiesConduct thorough, ongoing evaluations of elders’ physical, mental, and psychosocial needs in conjunction with interdepartmental staff to determine the appropriate level of support and care.Develop, implement, and continuously adjust individualized treatment and care plans based on elder progress.Connect elders with essential social, medical, housing, cultural, and vocational services to address barriers to care.Regularly communicate with clients through home visits, clinical check-ins, or facility meetings to ensure their rights and care needs are being met.Adjust treatment plans when status changes occur, and evaluate outcomes creating a continuation of essential services.Act as a liaison and advocate between the elder, their family/guardian, medical professionals, and social service agencies to ensure their rights and best interests are in continual alignment.Act as a community liaison and advocate between the elder and community based senior service agencies to ensure their rights and bests interests are in continual alignment with the care plan.Provide immediate support and navigate rapid-response resources for clients experiencing sudden emergencies or crises.Maintain accurate, detailed records of all elder interactions, progress notes, and services utilization while complying with legal and ethical privacy standards.Ensures compliance with all state, federal, local, and tribal regulations through accurate and detailed documentation and reporting methodology.Other related duties as assigned.Necessary Knowledge, Skills, and AbilitiesKnowledge and deep foundational relationships with tribal and local housing, public benefits (e.g., Medicaid, social security, senior resources), and mental health or substance abuse treatment programs.Knowledge of biopsychosocial dynamics of major illnesses, trauma, and diverse populations and behaviors.Knowledge and ability to interpret, follow, and implement tribal, local, state and federal laws and clinical reporting standards are required by funding sources.Ability to de-escalate emotional distress, conflicts, and guide adults through intermediate socioeconomic and psychiatric emergencies.Ability to provide adaptable verbal, written, and active-listening skills through an empathetic lens to mediate between elders, families, and multidisciplinary care or legal teams.Strong knowledge of Electronic Health Records (EHR) and computerized case management systems.Ability to manage a varying caseload efficiently, balancing field visits (e.g., home checks) with case reporting and office administration.Ability to establish authentic culturally based supportive report with vulnerable adults while promoting their autonomy, and personal goals.Ability to supervise, find educational training paths, ensuring development and team-based growth, allowing for programmatic development.Ability to make autonomous ethically sound decisions in situations of client crisis, and adapt elder care plans.Strong understanding and sensitivity to American Indian/Alaska Native (AI/AN) Tribal culture and lifestyle, working effectively with AI/AN populations.Effective oral and written communication skills with sensitivity to the lifestyles of targeted participants.Skills in operating personal computers using a variety of computer software.Ability to establish and maintain cooperative working relationships with management, coworkers, and outside contacts.Ability to maintain strict confidentiality and comply with applicable federal and tribal privacy laws and KIC policy (HIPAA, HITECH, etc.).Ability to learn and apply program/agency operating policies and procedures.Ability to manage multiple priorities and tasks concurrently and meet deadlines.Ability to work independently as well as with teams.Working ConditionsThe physical demands described here are representative of those that an employee must meet to successfully perform the essential functions of this job. This employee will be required to travel locally for community fieldwork regularly.
