Remote Case Manager — Elder & Disability Care (WI)

Humana

Inclusa is hiring compassionate case managers to support Wisconsin Family Care members. You will develop and monitor the Member Care Plan, coordinate with nurses, and perform visiting assessments at member homes.

Travel up to 40% is required for service delivery in Calumet, Outagamie, and Winnebago counties. Minimal qualifications include a Bachelor's in human services or related field and 1+ year experience with frail elders or disabilities, plus a valid driver’s license.
