Physical Therapist - Outpatient

Position Summary Physical Therapist - Outpatient Assumes full responsibility for evaluating a patient’s need for physical therapy, plans and executes individualized treatment programs in accordance with the State of Michigan licensure and PT scope of practice. Keeps treatment skills upgraded through continuing education, evidence‑based research, and program development. Responsibilities Provide patient care, treatment, and services within the scope of license, certification or registration and as required by law and regulation. Evaluate patient condition, establish treatment goals in collaboration with patient and family, and formulate written treatment plans. Delegate responsibilities to physical therapist assistants and rehabilitation technicians when appropriate; re‑evaluate patient status and adjust treatment plans. Order and instruct on medical equipment as needed, and participate in patient discharge planning. Document patient status and progress through evaluations, progress notes, and discharge summaries according to internal/external time standards and reimbursement guidelines. Communicate with interdisciplinary team members; attend conferences, meetings, and in‑services. Adhere to hospital policy and procedures, including patient confidentiality and infection control. Meet or exceed program productivity expectations and discipline‑specific coverage requirements. Support program operations, including orientation, training of new staff, clinical instruction for students, and program development or research activities. Meet or exceed clinical outcome collection standards and performance goals established by department leadership. Qualifications Graduate of Commission on Accreditation in Physical Therapy Education (CAPTE). Current licensure in every US state and territory where the therapist practices; licensure requirements must comply with state laws. Possesses and maintains BLS certification. Demonstrates knowledge of principles of growth and development over the lifespan and can assess data reflective of patient status to identify age‑specific needs.
