Licensed Mental Health Therapist Lumera Healthcare Lumera Healthcare is seeking a compassionate, motivated, and skilled Licensed Mental Health Therapist to join our growing outpatient behavioral health team. Key Responsibilities The Licensed Mental Health Therapist will: Assist with clinical intakes and assessments to help establish medical necessity and identify appropriate services. Participate in the treatment planning process from intake through discharge based on each client's diagnosis, needs, goals, and progress. Provide individual, family, and group therapy as clinically appropriate. Develop, implement, review, and update individualized treatment plans. Maintain complete, accurate, and timely clinical documentation in accordance with organizational, payer, regulatory, and professional standards. Ensure services provided are medically necessary, evidence-based, client-centered, and within the ethical and legal scope of practice. Monitor client progress and adjust treatment interventions as clinically appropriate. Coordinate care with physicians, case managers, community resources, and other service providers when appropriate. Participate in case consultations, clinical meetings, and interdisciplinary collaboration as indicated. Maintain professional boundaries and adhere to all confidentiality, HIPAA, ethical, and compliance requirements. Support continuity of care and appropriate discharge planning for clients.
