Physical Therapist - Per Visit, Home Health Care, Nassau/Suffolk locations

Position Summary We have an exciting opportunity to join our team as a Physical Therapist - PV, Home Health Care, Nassau/Suffolk locations. In this role, the successful candidate provides safe, therapeutic age‑appropriate physical therapy to promote improvement of functional mobility, physical ability, and quality of life. Uses clinical expertise to plan the delivery of care through examination, evaluation, planning, implementation and reassessment. Job Responsibilities Educates patient and family regarding patient’s functional status and plan of care. Provides a home exercise program and instructs on home safety factors. Re‑assesses patient progress toward short‑term and long‑term goals and modifies the plan of care accordingly. Completes and maintains accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention and evaluation according to Federal, Department of Health (DOH) and The Joint Commission (TJC) requirements. Assists agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participates in agency quality assessment and performance improvement initiatives. Performs other duties as assigned. Performs an objective functional evaluation and reassessment of patient including the use of appropriate tests and measures; evaluates patient safety issues and home environment factors; and establishes a plan of care with measurable and functional short‑term/long‑term goals. Implements and modifies appropriate interventions/treatments as indicated based on the patient’s functional status, diagnosis, age, patient response and status in conjunction with the plan of care. Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health‑care goals. Minimum Qualifications To qualify, you must have a Bachelor’s degree in Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program). Current and valid NYS Physical Therapy license and registration. 1–2 years experience in a hospital (inpatient), orthopedic (outpatient), or pediatric setting. Excellent communication (both oral and written), critical thinking, and customer service skills. Basic computer skills. Preferred Qualifications Master’s degree in Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program). Post‑doctoral/Doctorate in Physical Therapy (DPT). 1–2 years home‑care experience. Qualified candidates must be able to effectively communicate with all levels of the organization. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
