Job Description
Title: Physical Therapist - Rehabilitation

Schedule: Days

Assignment Details:
- Contract Weeks: 13
- Guaranteed Hours: 40

Requirements:
- Bachelor's degree required; Master's or Doctorate preferred.
- Active license as a Physical Therapist in applicable state.
- Basic Life Support (BLS) certification preferred.
- Prior experience in a long-term care or skilled nursing facility preferred.

Roles & Responsibilities:
- Evaluate residents and establish therapeutic plans of care with functional goals.
- Provide physical therapy services to maximize residents' functional abilities and independence.
- Monitor and evaluate treatment effects and modify plans as necessary.
- Document all interactions accurately in Electronic Medical Records.
- Collaborate with other healthcare providers to ensure comprehensive resident care.
- Mentor new staff and support the development of new rehabilitation programs and services.
