Your main responsibilities will be direct outreach, engagement, and tenancy support services to Oregon Health Plan (OHP) members through the Health-Related Social Needs (HRSN) program. Operating within MWVCAA's Community Resource Program and funded through the Coordinated Care Organization (CCO) for Marion and Polk counties, this position carries an active caseload, delivers trauma-informed and person-centered services, and works directly with members, landlords, and community partners to improve housing stability outcomes. This is a grant-funded position through November 2027, with potential extension contingent upon continued funding. This team offers a lot of growth opportunities and professional development through hands-on experience.Tuesday to Saturday. Day Shift. Flexibility Required. MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required.EDUCATION and/or EXPERIENCEHigh School Diploma or GED.Two years of case management or social services experience required.Experience with housing, sheltering, or homelessness-related programs strongly preferred.Equivalent combination of education and experience will be considered.Bilingual skill is required.CERTIFICATES, LICENSES, REGISTRATIONSDriver qualifications are required for this position. Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.Candidate must pass a comprehensive MWVCAA background screening prior to employment.Candidate must pass pre-employment and random drug screenings.CPR/AED and First Aid Certification required within first 30 days of hire.KNOWLEDGE, SKILLS, AND ABILITIESKnowledge of Housing First principles, trauma-informed care, and person-centered service delivery; understanding of social determinants of health and barriers to accessing services.Familiarity with OHP/Medicaid systems, HRSN program requirements, and Medicaid Waiver service delivery; knowledge of tenant rights and Fair Housing practices preferred.Proficiency in NeedNavigator, Unite Us, and Microsoft Office (Word, Excel, Outlook, Teams); ability to learn and adapt to new data and documentation systems.Strong organizational, time management, and documentation skills; ability to manage an active caseload, maintain accurate records, and meet deadlines in a fast-paced environment.Effective oral and written communication skills; ability to engage respectfully with diverse clients, landlords, community partners, and internal team members. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.Carries an active caseload of OHP-eligible members, providing Outreach and Engagement (O&E) case management support services and delivers Tenancy Support services once housing supports are authorized, including eviction prevention, housing navigation, rent and utility assistance coordination, and lease-related supports.Confirms OHP member eligibility for the HRSN Medicaid benefit; determines allowable services and calculates assistance amounts in compliance with PacificSource CCO and Medicaid Waiver requirements.Communicates directly with landlords and property management companies to collect required documentation, explain the HRSN program, and support members' ability to maintain housing.Enters timely and accurate data into NeedNavigator, Unite Us, and other required platforms; maintains complete and compliant documentation of all client contacts and service deliveries.Responds to inquiries from members, landlords, and partners in a timely and professional manner; escalates complex issues to the ARCHES Program Manager as appropriate.Participates in regular program meetings, trainings, supervision, and team collaboration; provides feedback to support continuous improvement.Adheres to all HRSN program guidelines, MWVCAA policies, and PacificSource CCO requirements; maintains professional boundaries with clients and coworkers at all times.Maintains consistent attendance and punctuality; some evening or weekend hours may be required.Provides coverage for the Eviction Prevention lobby as needed by welcoming clients, responding to inquiries, conducting initial screenings, directing individuals to appropriate staff or resources, and ensuring a positive and professional customer service experience.SUPERVISORY RESPONSIBILITIESThere are no supervisory duties in this position.PHYSICAL AND MENTAL DEMANDS Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties and responsibilities.Regulates emotions and maintains calm demeanor during interactions with clients who may be experiencing housing instability, trauma, or crisis situations.Manages pressures of a fast-paced, deadline-driven environment with frequent competing demands for attention and responsiveness.Specific vision abilities required include close vision and ability to adjust focus; manual dexterity for handling office equipment; occasionally lift up to 25 pounds.Occasional driving in all weather conditions to client sites, partner locations, and agency offices in Marion and Polk counties.WORK ENVIRONMENT Primarily indoor, office-based work environment with regular community-based work at client homes, partner sites, and housing provider locations.Some evening or weekend hours may be required; occasional remote work as approved by supervisor.Close quarters, often with a client population experiencing homelessness, substance use disorders, and/or behavioral health challenges.Occasional urgent situations may require coordination with law enforcement or emergency services.WHAT WE OFFER401k contribution after 1 year.Employee Assistance Program.Life insurance.Health, Vision and Dental Insurance.For some positions, employees may be eligible for company-sponsored continuing education.
