Physical Therapist (PT) – Shoreline Area

Location: Connecticut Shoreline
Compensation: $55–$65 per hour
Job Type: Full-Time / Part-Time / Per Diem
Service Area: Old Lyme to Madison, Connecticut

We are seeking a motivated and compassionate Physical Therapist (PT) to join our team and provide high-quality therapy services throughout the Connecticut Shoreline , covering the area from Old Lyme through Madison .

This is an excellent opportunity for a PT who enjoys building meaningful relationships with patients while providing individualized care in a community-based setting.

• Conduct comprehensive physical therapy evaluations.

• Develop individualized treatment plans based on patient needs and functional goals.

• Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, and functional independence.

• Monitor patient progress and modify treatment plans as clinically appropriate.

• Educate patients and caregivers on exercises, safety, mobility, and home programs.

• Coordinate care with physicians, nurses, care managers, and other healthcare professionals.

• Maintain timely and accurate clinical documentation.

• Communicate patient progress and concerns to the appropriate members of the care team.

• Provide services throughout the assigned Shoreline territory.

• Maintain compliance with Connecticut licensing and applicable healthcare regulations.

• Current and valid Connecticut Physical Therapist license .

• Graduate of an accredited Physical Therapy program.

• Current CPR/BLS certification, as required.

• Strong clinical, communication, and documentation skills.

• Ability to work independently and manage a flexible schedule.

• Reliable transportation for community-based assignments.

• Previous home health, outpatient, geriatric, or community-based experience is a plus.
