Certified Care Coordinator

To support patient care management, the full-time Certified Care Coordinator will conduct monthly outreach for a panel of 400-450 patients, providing health coaching and collaborating with clinical teams, all while working remotely from a HIPAA-compliant home office. Key responsibilities: Manage patient outreach and conduct daily outbound calls to ensure continuity of care Collaborate with clinical teams to address patient needs and support care coordination goals Document all patient interactions in real time while maintaining HIPAA compliance Required qualifications: Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician license Minimum 1 year of clinical experience in a healthcare setting Experience with Electronic Medical Records (EMRs) and strong computer navigation skills HIPAA-compliant home office setup with minimum internet speed requirements Must be legally authorized to work in the United States
