In this essential leadership role, you will be responsible for overseeing and directing case management operations to ensure the highest quality of care and optimal resource utilization.

- Lead and manage the Case Management department, including overseeing staff recruitment, training, scheduling, and performance evaluation.
- Oversee case management activities to ensure effective discharge planning, utilization review, and care coordination across the continuum of care.
- Collaborate with interdisciplinary teams to develop comprehensive care plans that meet patient needs while ensuring cost-effective care.
- Provide leadership during case conferences, fostering collaboration and proactive problem-solving to address complex cases.
- Manage day-to-day operations of the Case Management department, ensuring resources are utilized efficiently and processes run smoothly.
- A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing (MSN) or Healthcare Administration is preferred.
- Minimum of 5 years of clinical nursing experience, with at least 3 years in case management or care coordination.
- Previous leadership or supervisory experience in case management is required.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification is preferred.
