What You'll Be Doing Provide on‑site primary care and rounding services throughout assisted living, memory care, and long‑term care communities. Perform assessments, chronic disease management, preventative care, and follow‑up visits. Develop and adjust individualized care plans. Coordinate care with physicians, nurses, therapists, caregivers, and family members. Support transitions of care and help reduce avoidable emergency room visits and hospitalizations. Ensure continuity of care across a diverse patient population. Qualifications Active Minnesota Physician Assistant (PA-C) license in good standing. Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post‑acute care, or related settings preferred.
