Minimum Qualifications

• Current, active, and unrestricted Certified Nurses Assistant (CNA)
• Current BLS or CPR/First Aid certification
• Relevant nursing experience

Required Health Documentation

• Proof of immunizations:
• • Varicella
  • Rubeola (Measles)
  • Rubella
  • Mumps
• Physical examination or Statement of Health completed within the past 12 months
• Negative TB test within the past 12 months (or chest X-ray with supporting documentation)

Pre-Employment Requirements

• Clear background check (provided by Concentric)
• Negative drug screen (provided by Concentric)

Additional Requirements (Depending on Assignment)

• Arizona Fingerprint Clearance Card
• Crisis Prevention Intervention (CPI) certification
• PALS, ACLS, PEARS, and/or NRP certifications, as required by the facility
