Community Based Therapist

Position Summary Responsible for providing counseling and other clinical services to children, adults, couples and families in their natural environment. Essential Functions And Responsibilities Provides individual, group, and family therapy as needed in an office or out‑of‑office setting utilizing evidence‑based practices and a variety of therapeutic approaches. Out‑of‑office settings may include schools, homes, churches, and community settings. Completes diagnostic assessments and develops the Integrated Assessment and Treatment Plan with the client, parent/guardian, and/or caseworker. Follows all regulations when completing initial reviews and/or updates for all required paperwork. Provides crisis intervention services as needed. Participates in clinical case staffing as needed, scheduling staff to assure consistent communication with other professionals involved with the client. Collaborates with external agencies and programs as needed and with families to coordinate services on behalf of the individuals served. Maintains case records and notes in a timely and professional manner. Assures proper billing for all services provided, keeping up‑to‑date and professional records at all times and adhering to all payer regulations, including Medicaid Rule 132 and Managed Care Organization requirements. Participates in weekly supervision with a direct supervisor to explore areas of strength and growth. Completes all required reports and client documentation in a timely manner. Participates in Utilization Review of clinical files. Completes other duties as assigned. Qualification Requirements Strong organizational and time‑management skills. Excellent interpersonal and written communication skills. Must possess the experience, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines, and to provide services in a manner that reflects an understanding and appreciation of multiculturalism. Must possess and retain a valid driver’s license. Must possess and retain proof of auto insurance. DCFS/CANTS clearance. Education And/Or Experience A Master’s Degree in Social Work, Counseling, or Psychology from an accredited program, or in a related human service field; strong preference for a license in Clinical Social Work or Licensed Clinical Professional Counseling. The ability and willingness to actively participate in a fast‑paced, multidisciplinary team environment. Knowledge of evidence‑based practices and a variety of treatment modalities used with the population(s) served.
