• Software: HCHB experience required

Job Requirements

• Current Physical Therapist license in the state of Washington or eligibility to obtain one
• At least 1 year of recent home health physical therapy experience required
• Strong clinical skills with the ability to work independently in the home health setting
• Experience managing a productive caseload and meeting documentation timelines
• Proficiency with HCHB
• Excellent communication, time management, and organizational skills

Responsibilities

• Deliver high-quality physical therapy services to patients in their homes
• Evaluate patient needs and develop individualized treatment plans
• Monitor progress and adjust interventions to support patient goals and functional outcomes
• Maintain a minimum productivity expectation of 30 points per week
• Complete timely and accurate documentation by the next business day
• Collaborate with the care team to promote safe, effective, patient-centered care
• Travel within the assigned territory to serve patients across multiple counties
