Outpatient Physical Therapist

This is a 13-week contract position.

Monday, Wednesday, Friday: 8 AM - 5 PM Tuesday, Thursday: 9 AM - 6 PM

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patient needs, goals, and physician direction. The therapist will be responsible for delegating and directing discipline-specific therapy services and ensuring the design of functional treatment programs that promote effective returns to community activities. This role includes serving as an educator to patients, families, students, and coworkers.

Responsibilities:

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
• Document medical records clearly, concisely, accurately, and in a timely manner.
• Ensure care is provided under signed, valid physician orders in compliance with regulations.
• Participate in the student program, providing supervision and development of learning objectives.
• Create an individualized professional development plan and actively pursue continuing education.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure.
• Demonstrate awareness of performance improvement initiatives and contribute towards maximizing outcomes.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist.
• Previous assignments with the agency are highly desired; experience with Select Medical is a plus.
