Job Description

We are seeking a skilled Registered Nurse (RN) with experience in case management to join our healthcare team. The ideal candidate will play a crucial role in ensuring effective care coordination, utilization management, and discharge planning within an acute hospital setting.

Responsibilities

• Utilization management and care coordination for patients.
• Discharge planning and ensuring smooth transitions of care.
• Collaboration with healthcare teams to optimize patient outcomes.
• Revenue integrity and policy compliance management.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in a hospital setting.
• Active RN license in Compact State and New Hampshire at the time of submission.
• BLS certification required.
• Experience in acute hospital case management and critical care preferred.
