Job Description

We are seeking a skilled Registered Nurse (RN) with experience in case management to join our healthcare team. The ideal candidate will play a crucial role in ensuring effective care coordination, utilization management, and discharge planning within an acute hospital setting.

Responsibilities

Utilization management and care coordination for patients.
Discharge planning and ensuring smooth transitions of care.
Collaboration with healthcare teams to optimize patient outcomes.
Revenue integrity and policy compliance management.

Required Experience / Certifications / Licensure

Minimum of 2 years of experience in a hospital setting.
Active RN license in Compact State and New Hampshire at the time of submission.
BLS certification required.
Experience in acute hospital case management and critical care preferred.
