Continuum Therapy Partners is seeking an experienced and motivated Lead Therapist to join our rehabilitation team at Mineral Point Health Services in Mineral Point, Wisconsin.

This unique leadership opportunity combines direct patient care with clinical mentorship, program development, and operational support. We are currently considering licensed Physical Therapists (PT), Physical Therapist Assistants (PTA), and Occupational Therapists (OT) who are passionate about delivering exceptional patient care while helping build a collaborative, high-performing therapy team.

This position is based in Mineral Point with the opportunity to provide occasional coverage at our Lancaster location as needed.

Responsibilities

As the Lead Therapist, you will:

• Provide skilled therapy services within your licensed discipline.
• Evaluate and/or treat adult and geriatric patients in a skilled nursing setting.
• Serve as a clinical resource for therapists, assistants, nursing staff, and facility leadership.
• Assist with scheduling, team coordination, and communication across the rehabilitation department.
• Mentor therapists and therapy assistants while promoting collaboration and professional development.
• Support quality assurance initiatives, clinical programming, and regulatory compliance.
• Maintain accurate and timely documentation in accordance with company, state, and federal guidelines.
• Promote outstanding patient outcomes through evidence-based, patient-centered care.

Qualifications

• Graduate of an accredited Physical Therapy, Physical Therapist Assistant, or Occupational Therapy program.
• Current Wisconsin license as a Physical Therapist (PT), Physical Therapist Assistant (PTA), or Occupational Therapist (OT) required.
• Skilled nursing or rehabilitation experience preferred.
• Previous leadership, mentoring, or supervisory experience is a plus but not required.
• Excellent communication, organization, and leadership skills.
• Ability to collaborate effectively with interdisciplinary healthcare teams.
• Willingness to provide occasional coverage at our Lancaster facility when needed.
