Social Worker:

As a Social Worker with our team, you will have the opportunity to make a meaningful difference in the lives of our residents. You will work under the guidance of the Director of Social Services, attending care plan meetings and assisting with assessments and the MDS process to ensure our residents receive the highest quality care.

Key Responsibilities:

• Provide social and emotional support to our residents, helping to address their physical, emotional, and spiritual needs

• Assist with resident grievances and work to resolve any issues in a timely and professional manner

• Work closely with our multi-disciplinary team to develop and implement individualized care plans that meet the unique needs of each resident

• Participate in care plan meetings and contribute to the development of care plans that reflect the latest research and best practices

• Assist with the MDS process, ensuring that our residents receive the appropriate level of care and services

• Provide education and support to residents and their families on topics such as coping with loss, managing chronic illness, and navigating the healthcare system

Requirements:

• Associate's degree in Social Work (ASW)

• Part-time position with flexible scheduling to accommodate your needs (24 hours/week)
