Primary Care Physician - Conviva Care Center - San Antonio, Texas

Conviva Care Centers, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in San Antonio, Texas . You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach.

As a Primary Care Physician, you will report to our Associate Medical Director. This role will be onsite and in-person. We currently have several openings across San Antonio, including the following clinics:

\n - Conviva South Zarzamora 
\n - Conviva Live Oak
\n - Conviva Westover Hills
\n - Conviva Med Center

A day in the life of a Conviva physician

  Monday-Friday 8:00am-5:00pm

\n
• \n

  Join an experienced team with an established patient panel

\n
• \n

  See 12-15 patients per day . Opportunity to spend more time with patients

\n
• \n

  Longer authorized appointments (45 minutes)

\n
• \n

  Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care

\n

Required Qualifications

\n
• \n

  Doctor of Medicine or Osteopathic Medicine

\n
• \n

  Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine

\n
• \n

  Valid, unrestricted license in the state of desired employment (or willing to obtain)

\n
• \n

  Current, unrestricted DEA licens

\n

Preferred Qualifications:

\n
• \n

  Minimum of two+ years of experience

\n
• \n

  1+ years of experience managing Medicare Advantage panel of patients

\n
• \n

  Knowledge of Medicare guidelines and coverage.

\n
• \n

  Knowledge of HEDIS quality indicators.

\n
• \n

  Fluent in Spanish and English

\n
