St. Bernard Physical Therapy department is comprised of Inpatient Acute Care and Outpatient Mix. This role is primarily for the Outpatient setting. Therapists are welcome to float to acute care if interested. New graduates are welcome to apply! Hours of Operation Monday to Friday 8 am to 4:30 pm Optional to work weekend morning shift with pay differential. Location St. Bernard Ambulatory Care Center - 6307 S Stewart Ave, Chicago IL, 60621 Role Outpatient Mix (primarily orthopedics) Responsibilities Assess each patient and develop an appropriate treatment plan with measurable goals and time frames Provide physical therapy evaluation, treatment and follow up services to physician‑referred patient Collaborate with physicians to provide coordinated, holistic care 1 on 1 patient care Minimum Qualifications Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum Certification in Physical Therapy in a valid state Possession of a current/valid American Heart Association (AHA) Basic Life Support CPR Certification Pay and Benefits medical benefits, voluntary dental, vision, life insurance, tuition reimbursement, Retirement Savings plan #J-18808-Ljbffr
