Location: Miami, FL
Shift: Full Time
Pay Rate: $68,000–$80,000 annually (based on experience)

Join Our Team as a Wound Care Nurse (RN/LPN)

Azure Shores Rehabilitation is seeking a dedicated and compassionate Wound Care Nurse (RN/LPN) to join our clinical team. In this vital role, you will collaborate with an interdisciplinary team (IDT) to deliver high-quality wound and skin care services, ensuring optimal comfort, healing, and quality of life for our residents.

Key Responsibilities

• Collaborate with the interdisciplinary team (IDT) to plan, implement, and evaluate individualized resident care plans.

• Assist with completion of applicable sections of the Minimum Data Set (MDS).

• Review, update, and follow skin and wound care policies and procedures.

• Assess, treat, and manage primary and secondary wounds.

• Identify and manage risk factors for pressure ulcers and secondary wound infections.

• Perform routine skin assessments and assist with pain management related to wound care.

• Educate residents and families on wound treatments and care plans in a clear, compassionate manner.

• Evaluate the nutritional needs of residents with wounds and healing concerns.

• Recommend appropriate pressure-relieving devices, wound dressings, therapies, and products to the IDT.

• Promptly report any suspicion of abuse, neglect, fraud, waste, or crime to the Administrator or designee.

Qualifications

• Current, active, and unencumbered RN or LPN license in the State of Florida.

• Minimum of two (5) years of nursing experience; wound care experience preferred.

• Strong assessment, documentation, and communication skills.

• Experience in skilled nursing or long-term care is preferred.
