Occupational Therapist (OT) - Mobile Outpatient

Role Overview The Occupational Therapist provides outpatient rehabilitation services primarily in in-home settings for older adults, including private residences and independent living facilities. They assess, develop, and implement individualized treatment plans to enhance patients' functional abilities, focusing on areas such as mobility, balance, activities of daily living, and adaptive device use. The role involves supervising support staff, coordinating with healthcare teams, and ensuring high-quality patient care in a flexible, community-based environment. Key Responsibilities Evaluate and treat patients, documenting progress and adjusting plans accordingly. Collaborate with multidisciplinary teams and refer to other disciplines when needed. Order supplies, maintain equipment, and ensure a safe environment. Design and fit adaptive devices, instruct patients on their use, and train in functional activities. Perform routine reexaminations and support patient education and community reintegration. Qualifications & Skills Bachelor's degree in an ACOTE-accredited occupational therapy program. Initial certification from the National Board for Certification in Occupational Therapy. Valid, unencumbered state occupational therapy license. Reliable transportation or a valid driver's license.
