Licensed Practical Nurse (LPN)

Job Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our long-term care team. The ideal candidate will have a passion for helping others, with a strong focus on providing exceptional patient care. This role requires flexibility and the ability to float between different units as needed.

Shift Information

• Day Shift: 6 AM - 6 PM or 10 AM - 10 PM
• Schedule: 36 hours a week, including every other weekend

Key Responsibilities

• Provide high-quality nursing care to residents with common diagnoses such as dementia.
• Administer medications and treatments as prescribed.
• Monitor patient health and assess needs, reporting any changes to the healthcare team.
• Assist with daily living activities and ensure the comfort and safety of patients.
• Maintain accurate patient records and documentation.

Qualifications

• Must be a currently licensed LPN in New York.
• At least 1 year of experience in a long-term care setting.
• Basic Life Support (BLS) certification is required.
• Willingness to float between different units as needed.
• Expertise in working with dementia patients is preferred.

Patient Care Ratios

The ideal nurse-to-patient ratios are 1 to 20; however, ratios may reach up to 1 to 40 based on facility needs.
