Job Title: Registered Nurse - Case Manager
Location: 18300 US Highway 18 Apple Valley, CA 92307
Duration: 26 weeks
Shift: Day 5x8-Hour (08:00 - 16:30)

Summary
We are seeking an experienced Registered Nurse Case Manager for a 26-week travel assignment in Apple Valley, CA. The ideal candidate will have at least 2 years of acute-care case management and discharge planning experience, with strong skills in care coordination, utilization review, discharge planning, prior authorizations, and interdisciplinary collaboration.

The candidate must have hospital-based experience; behavioral health-only experience does not meet the requirement. Strong knowledge of working with HMOs/IPAs, Medicare regulations, DME coordination, SNF placement, ambulance transportation, and complex patient and family needs is highly preferred.

Requirements and Qualifications
· Active California RN license by start date.

· Minimum 2 years of acute-care Case Management/Discharge Planning experience.

· Previous hospital/acute-care experience required.

· Experienced traveler; first-time travelers are not accepted.

· Strong computerized documentation skills.

· Ability to independently manage a patient caseload.

· Must be willing to work every other weekend as required.
