Job Title: Family Practice Nurse Practitioner - Oregon (OR)


Location: Barlow, Oregon (97001) – Covering Lane, Multnomah & Washington counties


Start Date: 07/06/2026


End Date: 10/05/2026


Shift: Day, 5x8-hour shifts (08:00 – 17:00) – Monday through Friday, flexible scheduling; weekends and nights possible


Hours per week: 40 hours


Employment Type: Contract, month-to-month with possibility of extension


Licensure/Certifications: Active Oregon Medical License required; Medicard credentialing must be confirmed upon application


Experience: Previous primary care or home health experience preferred; comfortable seeing both pediatric and adult patients

What You’ll Do

• Conduct annual risk assessments and chronic condition reviews for diverse patient populations
• Perform assessment-only consultations for pediatric and adult patients
• Coordinate and escalate urgent findings to patients' primary care providers
• Manage in-home visits in Milwaukee area and telehealth appointments
• Follow scheduled appointments and maintain documentation
• Enter availability into Salesforce; ensure enrollment and credentialing in Medicard
• Support patient's health supervision through remote and in-home visits
• Promote patient safety and adherence to healthcare protocols

What You Need
Required:

• Oregon Medical License
• Experience in primary care or home health
• Comfortable with telehealth technologies and patient assessments
• Ability to perform both pediatric and adult evaluations
• Enrolled and credentialed in Medicard

Preferred:

• Previous telehealth experience
• Experience with risk assessments and chronic disease management
