Job Description
Title: Registered Nurse - Case Management

Schedule:
- Mon through Fri 8-5

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- Active Registered Nurse license in the State
- Case Management experience within the last 2 years
- Proof of Flu Vaccine required

Roles & Responsibilities:
- Provide professional nursing care across the lifespan in accordance with organizational standards and evidence-based practice.
- Assess, plan, implement, and evaluate patient care, delivering high-quality, culturally sensitive, patient-centered services.
- Collaborate with providers and interdisciplinary teams to promote optimal patient outcomes and safety.
- Conduct nursing evaluations and coordinate care plans with primary and specialty providers.
- Facilitate medical care and medication refills for Behavioral Health/Psychiatry patients, collaborating with physicians and pharmacies.
- Provide education and support to patients related to care and discharge needs; advocate for patient care within the healthcare system.
