About the Position

Job Title: Home Health RN Profession: RN Specialty: Home Health Duration: 13 weeks Shift: 12-hour Day Shift Hours per Shift: 36 Experience: Minimum 1 year of nursing experience; Home Health experience preferred License: RN license Certifications: BLS, driver's license, proof of vehicle insurance Must-Have: Homecare Homebase charting experience Critical thinking skills Experience with wound care, wound vacs, and IV infusions Description: Provide nursing care in home health settings. Manage and coordinate patient care for adults. Typical caseload includes 6-7 patients per day. Adhere to infection control practices and safety standards. Perform admissions and complete required documentation. Work with various patient diagnoses including diabetes and heart failure. Conduct assessments and develop care plans tailored to individual needs. Utilize Homecare Homebase for accurate charting. Demonstrate ability to drive to patient locations. Support patients with wound care, catheter management, and medication management. Display effective communication and collaboration with healthcare team members.
