Social Care Navigator: Connect People to Vital Resources

Human Development Services of Westchester, Inc.

Human Development Services of Westchester, Inc. is seeking a Social Care Navigator to help individuals access community resources, benefits, and services that improve health outcomes and support independence. The SCN works directly with clients to identify needs and social supports that promote stability, wellness, and independence, bridging gaps between clients and complex social service systems.
