Position Title: Adult Day Health Center Director

Reports To: Chief Nursing Officer (CNO), Chief Operating Officer (COO), and CEO

Department: Adult Day Health Services

Effective Date: June 2026

1. Qualifications (Education, Experience, and Personal Traits)

Education and Licensure:

· Current, unencumbered Licensed Practical Nurse (LPN) license in the State of Georgia (required).

· Graduate of an accredited practical nursing program.

Experience

· Minimum of three (3) years of nursing experience, with at least two (2) years in adult day health, long-term care, home health, or geriatric care preferred.

· Prior supervisory or management experience in a healthcare or adult day setting strongly preferred.

The Director must create and maintain a positive, engaging, and safe environment for clients while demonstrating respect for client dignity, protecting modesty, and maintaining clear boundaries. They must be punctual, follow all attendance and call-out procedures, and avoid negative behaviors such as poor attitude or discussing personal matters while on duty. The Director must foster teamwork, enforce policies consistently, and uphold the highest standards of care and compliance.

2. To Whom the Employee is Responsible

The Center Director reports directly to the CNO and is ultimately accountable to the COO and CEO. The Director provides daily supervision and leadership to Adult Day Health Aides, nursing staff, and other center personnel.

3. Duties and Responsibilities

Leadership & Staff Supervision

· Provide daily oversight and leadership to all Adult Day Health Center staff, ensuring high-quality care and alignment with company policies.

· Recruit, train, schedule, supervise, and evaluate Adult Day Health Aides and other team members.

· Conduct regular staff meetings, in-services, and performance evaluations.

· Enforce dress code, cell phone policy, attendance procedures, and professional standards.

· Assist with orientation and ongoing training of new caregivers.

· Address performance issues promptly with HR and maintain appropriate documentation.

Client Care & Safety

· Oversee the delivery of personal care, activities of daily living, health monitoring, medication reminders, and engagement activities.

· Ensure proper use of equipment (e.g., Hoyer lift) and client safety during arrivals, departures, transfers, and activities.

· Monitor client conditions, vital signs, intake/output, and report changes to the RN, physician, or appropriate parties.

· Maintain a clean, organized, welcoming, and odor-free environment.

· Lead emergency preparedness, drills, and response (fire, tornado, medical emergencies, etc.).

· Ensure compliance with all Georgia DCH licensing requirements, Medicaid waiver rules (SOURCE, CCSP, EDWP), and HIPAA regulations.

Operations & Administrative

· Manage daily center operations, including opening/closing procedures, meal service, nutrition oversight, transportation coordination, and facility maintenance.

· Complete required documentation, checklists, logs, and reports in a timely manner.

· Manage supplies, grocery needs, and inventory.

· Coordinate with the broader care team (RNs, therapists, case managers) for comprehensive client care plans.

· Participate in agency meetings, continuing education, and quality improvement initiatives.

· Provide oversite for programs like USDA, Medicaid Transportation, AAA Grant/other programs, etc.

· Perform other duties as assigned by administration.

Professionalism & Compliance

· Represent the company professionally at all times, modeling appropriate attire (scrubs preferred), visible badge, and conduct.

· Maintain strict HIPAA compliance and client confidentiality.

· Ensure the center remains organized, clutter-free, and compliant with all safety and regulatory standards.

4. Employment Overview

At-Will Employment Statement: Employment is at-will. Either the employee or Family Care, Inc. may terminate the employment relationship at any time, with or without cause or notice.

Resignation: Due to the critical leadership nature of this position, we strongly encourage at least four (4) weeks’ written notice to ensure continuity of operations and client care.
