This role partners closely with medical/psychiatric providers to deliver integrated, patient-centered behavioral health services directly in patients' homes, assisted living facilities, and other community settings.This position is ideal for clinicians who are highly independent, organized, and comfortable working with medically complex older adults. The counselor is responsible for managing their own schedule, providing in-person counseling services, and ensuring all documentation meets Medicare and regulatory compliance standards.Providers are independent contractors. Compensation is fee-for-service, based on completed visits and submission of compliant documentation. Required LicensureCandidates must hold an active, unrestricted license in one of the following:Licensed Professional Counselor (LPC) Licensed Clinical Social Worker (LCSW)Clinical Psychologist (PhD or PsyD) Key ResponsibilitiesGeriatric Behavioral Health CareProvide individual therapy and supportive counseling to geriatric patients (primarily Medicare population)Address common conditions such as depression, anxiety, adjustment disorders, grief, cognitive decline, and chronic illness-related behavioral health concernsConduct comprehensive psychosocial and behavioral health assessmentsDevelop and implement individualized, goal-oriented treatment plansUtilize evidence-based, age-appropriate therapeutic interventionsIntegrated Care CollaborationWork closely with mobile primary care and psychiatric providers to support whole-person careCommunicate clinical findings and collaborate on care plans for patients with complex medical and behavioral needsSupport caregivers and family members with education, coping strategies, and care planningMobile Service DeliveryProvide care in patients' homes, assisted living facilities, and group homesAdapt clinical approach to non-traditional treatment environmentsMaintain professionalism, safety awareness, and flexibility in the field Scheduling & ProductivityIndependently create, manage, and maintain a consistent weekly scheduleCoordinate directly with patients, caregivers, facilities, and referral sourcesOptimize scheduling routes and volume to maximize productivity and continuity of careMultiple providers will be referring for steady influx of patients Documentation & Medicare ComplianceComplete timely, accurate, and defensible clinical documentation for every visitEnsure documentation supports medical necessity in accordance with Medicare guidelinesMaintain compliance with: CMS regulationsHIPAA requirementsInternal policies and quality standardsSubmit documentation promptly to support billing and revenue cycle operations CompensationFee-for-service modelPayment based on: Completed patient visitsTimely submission of compliant documentationNo guaranteed salary; compensation is directly tied to productivity and efficiency  QualificationsActive, unrestricted license (required)Experience working with geriatric or medically complex populations strongly preferredKnowledge of Medicare documentation standards and medical necessity criteria preferredExperience in home-based, community-based, or integrated care settings a plusStrong clinical, organizational, and independent decision-making skillsReliable transportation and willingness to travel within assigned territoryProficiency in EMR/EHR systems Key CompetenciesSelf-directed and highly accountableStrong time management and route planning skillsClinical confidence working independently in the fieldExcellent communication and interdisciplinary collaborationCompassionate, patient-centered approach to elderly populationsCommitment to compliance, quality, and ethical practice Work EnvironmentFully mobile, field-based positionFlexible schedule determined by the clinicianHigh level of autonomy with support from clinical and administrative leadershipCollaborative, team-based care model integrated with primary care and psychiatric providers All Aegis Healthcare employees must be able to pass a criminal background check.
