Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP)

The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position. Other duties may include but not limited too:

• Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
• Demonstrates positive, practical communication skills and professional behaviors that promote cooperation and teamwork with patients and other customers.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with the interdisciplinary team.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and consults to specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
• Patient intakes - vital signs/ekg acquisition/screen symptomatology.
• Take and manage patient phone calls, medication renewals, triage issues.
• Medication titration for CHF, lipids and HTN management according to guideline directed medical therapy criteria (GDMT)
• Review of laboratory results, identification of abnormalities and communication of results to both the provider and the patient in a timely manner.
• Patient education for procedures and medications.
• EKG and Event Monitor review and interpretation.
• Stress Test Supervision (physical and pharmacological) and review and interpretation of results.
• Patient care that is compassionate, appropriate, and effective for treating health problems and promoting health
• Identify barriers to learning and prioritize education for cardiovascular disease.
• Utilize concepts of Motivational Interviewing when counseling patients.
• Provide emotional support to patients, families, and caregivers.
• Know the methods to evaluate and manage cardiovascular conditions and diseases including but not restricted to hypertension, CHF, valvular heart disease, coronary artery disease, dysrhythmias, in diverse patient populations that are considered in the realm of current cardiovascular care.
• Monitor and manage anti-arrhythmic drug (AAD) therapy. Skills to perform a cardiovascular risk assessment and to develop an evidenced based treatment plan.
• Skills to educate patients with cardiovascular disease on a heart healthy diet regimen.
• Contributes to performance improvement initiatives by identifying, analyzing, developing, and implementing processes that improve care at the panel, program, or population level.

Pay: Competitive salary, regular salary increases, potential for performance awards
