Gotham Enterprises Ltd is looking for a dedicated and qualified Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity, specifically within our Hospice Center services based in Fort Worth, Texas. Work Environment
This is a remote position allowing you to work from the comfort of your home while still providing essential support to clients in our community. The immersive environment requires a proactive approach, as you will be interfacing with various stakeholders, including healthcare professionals, patients, and families, from a distance. Effective communication and collaboration tools will be utilized to ensure seamless service delivery and coordination of care.
Key Responsibilities
- Deliver direct clinical services to assigned clients, conducting assessments to identify symptoms of major mental illness and severe emotional disorders.
- Collaborate closely with the healthcare team to develop integrated care plans that reflect the individual needs of clients, facilitating access to a variety of linked services.
- Provide individual and group therapy sessions, employing evidence-based practices to promote recovery and empowerment.
- Monitor patient progress, ensuring adherence to treatment goals and documenting outcomes accurately.
- Participate in crisis intervention as needed, offering timely support and interventions during emergencies.
- Engage in regular check-ins with clients, families, and other providers to maintain continuity of care.
- Create and implement brief, focused prevention and treatment services that address immediate concerns and enhance overall wellbeing.
- Maintain a caseload effectively, ensuring all client interactions are documented according to compliance standards, including the timely submission of relevant reports.
- Contribute to quality improvement initiatives by participating in committees aimed at enhancing service delivery and operational efficiency.
Patient Population / Client Focus
In this role, you will be working primarily with patients in hospice care who are experiencing a range of mental health challenges alongside life-limiting illnesses. Understanding the complexity of their circumstances and cultivating a strong therapeutic alliance are critical to delivering effective care. You will play a key role in addressing the emotional and psychological needs of these individuals, ensuring they and their families receive the support they require during this sensitive time.
Qualifications
- A Bachelor’s Degree in Social Work, Counseling, Psychology, or a related field is required; a Master's degree is preferred.
- Must possess a valid state-issued license as a Licensed Clinical Social Worker (LCSW).
- Demonstrated experience working in mental health settings, preferably with diverse and under-served populations.
- Strong understanding of ethical guidelines and standards for professional conduct in social work.
- Excellent communication skills, with the ability to engage effectively with clients, families, and interdisciplinary teams.
- Proficiency in using electronic health records (EHR) and other relevant clinical documentation systems.
Preferred Experience
- Previous experience in hospice or palliative care settings.
- Familiarity with crisis intervention techniques and procedures.
- Background in implementing evidence-based treatment modalities.
- Experience conducting assessments and developing care plans tailored to individual client needs.
Compensation and Benefits
