Community Health Worker II: Care Transitions & Navigation
Cambridge Health Alliance
The CHW II at Cambridge Health Alliance will collaborate with the Complex Care Management team, primary care and inpatient teams to provide short-term support for patients transitioning from the hospital or post-acute settings back to the community. You will assess discharge plan adherence, identify barriers, conduct health-related social needs assessments, and ensure follow-up appointments. You will refer higher-risk patients to CCM when needed and follow MA practice guidelines. #J-18808-Ljbffr Cambridge Health Alliance
